One of the most common reasons for visits to our office in Kingwood is
sciatica.<span style="mso-spacerun: yes;"> </span>However, sciatica is not
actually a diagnosis but a description of symptoms.<span style="mso-spacerun: yes;"> </span>The term sciatica is well recognized and
popular, so it is often overused and inaccurate, similar to the term Carpal
Tunnel. Sciatica can include any symptoms related to the compression of any one
of the five nerve roots (from the spine) that make up the sciatic nerve.<span style="mso-spacerun: yes;"> </span>It gets used a lot for any back, leg or butt
pain.<span style="mso-spacerun: yes;"> </span>There are many different causes of
these symptoms and not all patients with these symptoms have sciatica.<span style="mso-spacerun: yes;"> </span>Getting to the root cause is important in
treating sciatica and not just treating the symptoms with pain killers, etc.</div>

When somebody comes in with symptoms of sciatica, the next step
is evaluating the actual cause.<span style="mso-spacerun: yes;"> </span>This
often includes orthopedic and neurological tests with x-rays to evaluate the
spine and pinpoint the potential cause instead of just treating the symptoms.<span style="mso-spacerun: yes;"> </span>After a thorough exam and evaluation then it
can be determined if an MRI is necessary for further evaluation.<span style="mso-spacerun: yes;"> </span>In most cases it is not and the condition
that is causing the symptoms responds well to non-invasive conservative
chiropractic care.</div>

So what are the causes?<span style="mso-spacerun: yes;">
</span>Potential causes of sciatica symptoms include spinal disc herniations,
piriformis syndrome, pregnancy, spinal stenosis, localized nerve entrapments,
trauma, or in rare instances, tumors.<span style="mso-spacerun: yes;">
</span>Spinal discs over time can began to bulge and herniate causing compression
of the nerve at that level.<span style="mso-spacerun: yes;"> </span>It can happen
anywhere along the spine, but the most common level is at L5-S1 (or the last
disc in the spine) and can cause pain down the back of the leg.<span style="mso-spacerun: yes;"> </span>Surgery for these cases is a last resort and
should not be considered unless not responding to conservative care.<span style="mso-spacerun: yes;"> </span>Piriformis syndrome is compression of the
sciatic nerve by the piriformis muscle.<span style="mso-spacerun: yes;">
</span>This usually responds very well to conservative care and includes
evaluation of the <b style="mso-bidi-font-weight: normal;">causes</b> such as leg
length, spinal and pelvic rotations, muscle imbalances and postural
modifications.<span style="mso-spacerun: yes;"> </span>Pregnancy may cause
symptoms of sciatica from the weight of the fetus on the sciatic nerve,
swelling or piriformis syndrome.<span style="mso-spacerun: yes;"> </span>Spinal
stenosis is narrowing of the canal that the spinal cord passes through from
usually a combination of factors that may include bone spurs, disc herniations,
inflammation, spondylisthesis (forward displaced vertebra) and spinal ligament
thickening.<span style="mso-spacerun: yes;"> </span>Some severe cases of spinal
stenosis may require surgery.<span style="mso-spacerun: yes;"> </span>Other
localized nerve entrapments can occur in the legs that can mimic sciatica
symptoms and often respond very well to conservative non-pharmalogical
treatment.</div>

Every patient’s structure is different and no two cases are
exactly the same, so the treatment of sciatica varies from patient to patient
due to these factors.<span style="mso-spacerun: yes;"> </span>Live well and God

At our office in Kingwood, we often get patients that complain of lower back pain when running on a treadmill. Why is this? Although treadmills are supposed to simulate running outdoors, the fact is you are not really going anywhere and are going to create a greater impact on the spine. We find that many of the patients that complain of lower back pain after running on a treadmill have what is called Lumbar Facet Syndrome. Often on x-rays of the lower back, we will find increased sclerosis (calcium buildup or arthritis) in the joints of the lower back. <br />
<br />
So what do we do about it? <br />
<br />
First, it is best to switch it up. Get outside and run/walk. This changes up the stress on the spine and allows you to use different muscles that aren't used on a treadmill. Another problem with treadmills is that you are always going straight and never turning. When running outside, because of turns, etc., more muscles get used and thus is a better workout. <br />
<br />
Second, incline it! Adjust the settings on the treadmill so that it is at a slight incline (~3 degrees or more). This helps limit the stress and impact on the joints of the lower back by changing the angle of the pelvis when running.<br />
<br />
Of course, always get your back properly evaluated by your chiropractor first to make sure it is not a more serious issue. We usually see quite of few injuries from doing yard work, especially the lower back. Here are a few tips to prevent injuries:<br />
<ol><li>Stretch - Stretching before doing yard work is just as important as before sports.</li>
<li>Keep your nose between your toes - Make sure to use your legs and avoid repetitive bending and turning to the side. This creates a lot of stress to the lower back and the discs.</li>
<li>Hydrate - Drink plenty of water to keep your muscles hydrated.</li>
<li>Split it up - Don't try to conquer the entire yard in one day. Also, change it up. For example, spread some mulch, then mow, then sweep, then do some more mulch, etc. Don't do the same activity for prolonged periods of time.</li>

One of the keys to a healthy lifestyle is regular exercise. I have heard all the excuses and use them myself at times. I know you are busy and there are a lot of demands on your time. However, I know it makes life even harder if you are trying to get everything done that needs to be done but are not functioning at your optimum potential because of an unhealthy lifestyle.<br />

One way to improve your fitness at work is to take advantage of the opportunities that are available to you on a daily basis. Opt for those activities that demand more effort over those that are more convenient. For example, use the stairs whenever possible. If it is an option, use your own horsepower to get to work via running, walking, or biking. Or park as far as possible when going to work or the store. Keep it as a part of your consciousness to move as much as possible. Not only will it help you burn calories it will help you stay more alert and focused.<br />

Finally, with today’s smart phones, there are some really cool, free apps to help you improve your fitness at home and even at work. One really good one is Workout Trainer on Android. This app will suggest free workouts based on your time restraints, intensity desired and requires little to no equipment. Another good app for running, walking and biking is iMapMyRun which uses GPS to determine and display your distance, time, pace, and calories burned. There are other many good apps out there and they are constantly getting better.<br />

These are just a few ideas to get your wheels turning. I encourage you to start coming up with some of your own ideas of how you can become more active in your work environment. This will be a great start towards a healthy lifestyle that will help you get more years out of your life and more life out of your years.<br />

Each year, Azleway brings hope and opportunity to more than 1,000 Texas children at-risk and in crisis. For children of Azleway, Christmas can be a very hard time. Although most of the children of Azleway have been removed from homes full of abuse and neglect, Christmas is still a time when children miss home and family, no matter how destructive the environment may have been. <br />

Last month we talked about Nutritional Management of Arthritis. Now, let’s focus on what can be done physically to treat arthritis. You may think that if a joint is arthritic you should try to rest it as much as possible but that is not really accurate. What would work better, a rusty old hinge that gets used regularly every day or one that hasn’t moved in a year?<br />
<br />
Obviously there are many factors that go into what type of activity you should do. The type of arthritis, the location and the severity are all factors that need to be considered. Talk to your chiropractor about your specific case. That being said there are many options for people with arthritis when it comes to exercise. One of the most important things is to maintain an active lifestyle. Either in your work, your leisure time or preferably both involve yourself in activities that require you to get up and move.<br />
<br />
When it comes to exercise start off nice and easy and let your chiropractor know of any changes that you notice as a result of your new exercise program. We suggest that you start off with something that focuses on flexibility and movement such as yoga. Gradually, you can increase your workouts to include cardio and strength training. Swimming and biking are great low impact cardio exercises that can help keep you limber while strengthening the heart and lungs thereby promoting overall health. <br />
<br />
Your strength training options are only limited by your imagination. If you need help coming up with some exercises talk to your chiropractor or find a qualified personal trainer to help you get started. Not only will strength training help with your arthritis it will strengthen your bones and muscles which also helps protect your joints.<br />
<br />
If you lack the discipline necessary to exercise regularly look for activities that you can do outside of your daily routine to get your heart pumping and your body moving. Dancing is an excellent example of an activity that incorporates both. Go out dancing, take a class or check your local events calendar for dances. Thanks to Dancing With the Stars, ballroom dancing has become popular.<br />
<br />
Last but not least is getting adjusted regularly. If a joint is locked up or subluxated it can’t move in its normal range of motion. A chiropractic adjustment frees up the joint and removes stress from the joint and nervous system. This allows the joint to move more freely and allows the brain to better understand the environment and adapt to changes in the environment.<br />
<br />
So get moving. Talk to your chiropractor, find activities that you enjoy and are appropriate to your health status and get to it. The role that diet plays on both RA (Rheumatoid Arthritis) and OA (Osteoarthritis) has been studied for over 80 years. Up to this point there is little consensus among researchers as to the role of nutrition and supplementation regarding both of these conditions. In this article I will list some of the most recommended and effective approaches.<br />

In all the research that has been done on arthritis and nutrition, two things are clear. First, for osteoarthritis, consuming a diet that allows you to maintain a healthy weight is paramount. As I have mentioned before, each pound of body weight increases exponentially the stress on weight bearing joints, thus increasing the risk of developing OA. For both conditions, maintaining a balanced healthy diet is important. Remember the golden rule in nutrition, if you put garbage in you should expect to get garbage out. You can’t expect to consume poor quality foods and expect your body to turn a Twinkie into high quality tissue… <br />

Shoulder pain is quite common and when you look at the anatomy of the shoulder it is pretty easy to see why. The design of the shoulder predisposes it to injury with overhead activities. Many shoulder problems begin with either repetitive overhead activities such as painting, cutting hair, assisting dentists, lifting weights, putting on makeup, pitching a baseball, cutting limbs from a tree, or swimming. Of course the list could go on and on, but you see what I am talking about. <br />
<br />
So the first tip is to avoid overhead activities as much as possible and when raising your arms, try not raise them above shoulder level. We treat a lot of people who lift weights and workout on a regular basis, and the first thing I tell them when they come in with shoulder problems is that I will not take them as a patient if they continue to do military press and incline press. They usually don't like to hear that, but the fact of the matter is that they will not get better if they continue to do those exercises. I learned the hard way when I strained the rotator cuff in my left shoulder back in college doing incline press with a straight bar. Once their shoulder is healed, I will then show them what I call a modified military press that they can do without aggravating the rotator cuff.<br />
<br />
The second tip is to use ice packs over the shoulder if there is significant pain or swelling. Never use heat when there is swelling. That's why you always see professional baseball pitchers with big ice packs on their shoulders in the dugout after pitching, to keep the swelling down.<br />
<br />
The third tip is that if it is not getting better to get it properly evaluated. Just getting diagnosed with Rotator Cuff Syndrome doesn't really give you the full picture of what is truly going on. Every condition can be different and after thoroughly evaluating the shoulder we can then give personalized care and instructions.<br />

Well, that's what I call it. What is it you ask? It is 99.9% of the time found on the right side. It is located between the shoulder and the neck. It can sometimes lead to neck pain, headaches and presents itself usually as a tender knot just above the tip of the shoulder blade.<br />
So, why do I call it Mouse Shoulder? I call it Mouse Shoulder because the most common factor leading to this epidemic is the use of the computer mouse. Mouse Shoulder is joining the ranks of Carpal Tunnel, Trigger Finger, and Elbow Tendinitis as common problems caused by computers. Most people typically spend hours a day on the computer and almost always use there mouse on the right side. (I have met one person who switches back and forth between her right hand and her left hand, but that's just wierd) The repetetive use of the mouse causes significant built up tension in the right shoulder and neck region leading to Mouse Shoulder. <br />
So how do you prevent it? The easiest way to lessen the stress caused by the mouse is to make sure that you are keeping it close to you and that you don't have to reach for it. The reaching is what causes the tension in the neck and shoulders. Often times a cordless mouse may be needed to keep the mouse close to you.<br />

Here is some advice.A question we often get asked is “What type of mattress do you recommend.” And the answer can be different for each individual. If you are sleeping on a 20 year old mattress then it is most likely time to look for a new one. There are a lot of different types of beds to choose from today and sometimes it can be overwhelming. There are coils, memory foam and even beds with air chambers (stay away from water beds). They can also range pretty significantly in price. <br />
<br />
So here is how I feel. There are studies that show this and studies that show that, but it comes down to can you sleep comfortably with proper support and without pain. Ideally you want a mattress that gives your spine support and is not too soft. If it is too soft, this can be bad on the curvatures, discs, and ligamentous structure of the spine. If it is too hard, it may be too uncomfortable to sleep on. For most back sleepers I would recommend a medium firm mattress without a pillow top or very small pillow top. It is also worthwhile to compare the number of coils from mattress to mattress and the gauge (thickness) of the coils for durability. For most side sleepers, I would again recommend a firm mattress, but with a slightly thicker pillow top than that of back sleepers. The reasoning is that if a mattress is too firm for side sleepers it may irritate the shoulders and hips, thus aggravating and even causing a bursitis (which really hurts). The other types of mattresses like memory foam (ex. Tempurpedic) and air chambers (ex. Sleep Number) can be good also, but are usually much more expensive. Not all memory foam mattresses are built the same, so make sure to do your research on the brand and its durability. Most of the patients I know that have foam mattresses really like them. The only complaints I usually hear is that they can tend to be a little warm. The air chamber beds are good in theory because you can adjust the firmness on each side for each individual, since husband and wife tend to be different sizes. However, the problem is that what may feel like the most comfortable setting may not be best for that person’s spine and can make it worse. I have heard mixed reviews from patients with this type of bed. <br />

When buying a mattress, it is a good idea to buy from someone that has at least a 30 day trial guarantee since a mattress will feel great in the store, but after sleeping on it a whole night may not feel so good. All mattresses are going to wear over time (no matter what the cost) and need to be replaced. So to make a long story short, I recommend a firm, but comfortable mattress. Very cool. They were energetic and moving with purpose. So age can be just a number......with the right preparation. One thing I noticed which can be very helpful for those wanting to be like these ladies is that they were keeping each other accountable. I overheard them discussing what their plan was for the morning. They discussed how many reps, how much weight and in what order they would be doing the exercises. This can help keep you motivated with a purpose. They also were being safe and lifting the weights with the proper, technique, posture, etc. So a few tips that may help make your New Year the beginning of a new life is to find a workout partner to keep you accountable, plan your activities, find out from your chiropractor which exercises you should refrain from, perhaps hire a trainer for a few sessions to learn proper technique and how to use the equipment, have clear cut goals and above all have fun! However, there are many different conditions that can cause similar symptoms and actually, true Carpal Tunnel is pretty rare. Other conditions such as Thoracic Outlet Syndrome, Cervical Disc herniations, and cardiovascular problems are very common and can often get missed diagnosed as Carpal Tunnel. Someone with these symptoms should be properly evaluated by an expert in neuromusculoskeletal disorders, such as your chiropractor, to get a proper diagnosis and treatment.<br /><br />Most true Carpal Tunnel can be treated conservatively without the need for drugs or surgery. Some like the idea of having government get involved to help insure everyone and others want the government to stay out, fearing socialized medicine. Healthcare costs are escalating and will continue to escalate. The problem is that most of us are not getting the big picture.<br /><br />The problem with healthcare is the health of the country. For example, over 70% of healthcare expenses are for the treatment of lifestyle diseases, such as, diabetes, heart disease, and cancer. Many people blame genetics. However, even though a person may be genetically susceptible to certain conditions, in most cases it is the lifestyle behaviors that bring out that genetic expression. Currently, 80% of people over the age of 25 are overweight. According to a UCLA study, obesity increases a person’s yearly healthcare costs by an astounding 36% and medication costs by 77%. According to the Surgeon General, in 1996, obesity cost us $117 billion and he warned then of an impending epidemic of obesity, caused primarily by industrialization and sedentary lifestyles, that would lead to a healthcare crisis. Can you imagine what the cost is now?<br /><br />The health of the people continues to decline and as long as that continues, healthcare costs will continue to rise no matter what plan is put forth. We will either pay for it in insurance premiums or taxes. I always say, “The best way to lower your healthcare costs is to get healthy.” The best way to get healthy is to change the habits that made us unhealthy. One problem is that most of us don’t truly understand what it takes to become healthy. However, there is no money in educating people on ways of improving their health, big pharma is not going to pay for it, and honestly from experience in my office, most people are not interested in educating themselves unless it is affecting their lifestyle. For example, a while back I did a free workshop in my office on osteoporosis prevention. Unfortunately, the only people who attended where women who already had osteoporosis. This is very disappointing. First of all, the information is free. Second, the people attending should have been young adults looking to lower their future healthcare costs by decreasing their future dependency on costly medicine.<br /><br />I believe we need to help the sick and take care of them; however people also need to be held accountable for their health. Healthy people should not incur higher insurance premiums or taxes, because others are not taking care of themselves. It’s amazing to hear someone complain about the high cost of healthcare, then go and “supersize” their next meal. That is ridiculous. Since money talks, maybe we should reward people who are healthy with tax breaks for motivation. As a society, we should be driven to improve our health not only for our own sake, but for the future of our children and the enormous financial burden we will leave them with.<br /><br />I challenge everyone to take a serious look at their health and find methods of improving it. Take advantage of opportunities to educate yourself on these methods. Find someone knowledgeable about health to act as your healthcare coach for advice, counseling and direction. And finally, empower and encourage others to do the same.
